  • Complete this only after submitting your SureLC request
  • Fill in all required information accurately
  • Once completed, your contracting request will be submitted to American Amicable, SBLI & Banner Life for processing
  • Carriers will review your request and notify you directly of their contracting decision via email

Step 2: Wait for Contracting Invitation via Email

  • Complete the contracting application, review all questions, sign and complete
  • Once completed, your contracting request will be submitted to UHL for processing
  • UHL will review your request and notify you directly of their contracting decision via email
