Step 1: Complete the Contracting Request on SureLC

Step 2: Complete Google Form
Section titled “Step 2: Complete Google Form”- Complete this only after submitting your SureLC request
- Fill in all required information accurately
Step 3: Carrier Processing
Section titled “Step 3: Carrier Processing”- Once completed, your contracting request will be submitted to American Amicable, SBLI & Banner Life for processing
Step 4: Carrier Notification
Section titled “Step 4: Carrier Notification”- Carriers will review your request and notify you directly of their contracting decision via email
For Occidental Life
Section titled “For Occidental Life”Submit a request ONLY if you have business to submit.

Step 2: Wait for Contracting Invitation via Email
