Submit a request ONLY if you have business to submit.

Step 1: Complete the Contracting Request on SureLC

  • Go to the SureLC Portal
  • Sign in or create your SureLC account
  • Complete your contracting request:
    • Select OneAmerica as the carrier
    • Make sure all your information is up to date, including your E&O insurance, license(s), and signature

Complete the Google Form only after submitting your SureLC request.

Fill in all required information accurately:

  • Full name
  • Email address
  • Any additional business details as requested

Once completed, your contracting request will be submitted to OneAmerica for processing.

OneAmerica will review your request and notify you directly of their contracting decision via email.

  • Processing Time: Allow 24-48 hours for GFI processing and a full 7-10 business days for carrier to process appointment paperwork
  • Status Inquiries: Email appointmentstatus@mygficonnect.com for updates
  • Next Steps: Keep an eye on your email for next steps to finalize your carrier appointment

Important: SureLC + Google Form required (complete in order). This process is for LTC (Long-Term Care) contracting. Google Form must be submitted AFTER SureLC request.
