Step 1: Complete the Google Form

Step 2: Wait for Contracting Invitation
Section titled “Step 2: Wait for Contracting Invitation”- Invitation will be sent to your email after the form is received
- Complete the contracting application, review all questions, sign and complete
Step 3: Carrier Processing
Section titled “Step 3: Carrier Processing”Once completed, your contracting request will be submitted to EquiTrust for processing.
Step 4: Carrier Notification
Section titled “Step 4: Carrier Notification”EquiTrust will review your request and notify you directly of their contracting decision via email.

Section titled “Important Processing Notes:”- Processing Time: Allow 24-48 hours for GFI processing and a full 7-10 business days for carrier to process appointment paperwork
